Registration Form

Please print and complete the registration form and return it to First Presbyterian Church. When your completed registration form is received, your child will then  be added to the waiting list. Children are added to the waiting list in the following order: First Presbyterian Church members, children already in CDO or with siblings in CDO, followed by new applicants.
